For all Models that reside in
the USA You Will need 2 Forms of ID
Acceptable
forms of ID for your first form of Identification:
You must provide a Government Issued pictured ID in
one of the following categories:
*Passport
*Driver’s License
*State Issued I.D.
*Military I.D.
All models must fill out the model release and provide the proper identification with the model release and the model is
required to fill out the 2257 Compliance form to be accepted.

fill out the Agreement Release form
as well as a 2257 Form and 2 Forms of ID for Both)
After you fill out and print make sure you
attach 2 forms of ID for each form and send to:
